- Meet the criteria for the service

CONFIRM THE CLIENT HAS GIVEN CONSENTStep 3
SUBMIT THE REFERRAL TO THE ONE YOU KENT TEAMORGANISATIONS THAT MAY REFER
Referrals are welcomed from:
- GP practices
- Pharmacies
- Hospitals
- Community organisations
- Housing providers
- Social prescribing teams
- Voluntary sector organisations
