It’s a conversation where we listen to you and create a personalised plan that works for your life.

Together we create a personalised plan that fits your lifestyle and supports you to make realistic changes.

Step 1

Your Story

Your journey begins with Your Story.

This is a conversation with a trained lifestyle advisor where we listen to you and learn about your experiences, goals and challenges.

During this conversation we may talk about:

  • Your current lifestyle
  • What you would like to change
  • Your health goals
  • The type of support that would work best for you

Step 2

Your Personal Plan

Based on Your Story conversation, your advisor will help you choose the support that works best for you.

This may include:

  • Stop smoking support
  • Healthy weight programmes
  • One-to-one support
  • Group sessions
  • Phone or online appointments

Step 3

Ongoing Support

Your advisor will support you throughout your journey.

Together you will:

  • Set realistic goals
  • Track your progress
  • Adjust your plan if needed
  • Celebrate your achievements

If you live in Kent and want support to stop smoking or manage your weight, you can refer yourself to One You Kent.

To complete the referral form you will need:

  • Your postcode
  • The type of support you would like
  • Some basic health information
